About the Health Fund

The MCASF Local 725 Health & Welfare Trust Fund is a healthcare plan. The Plan most recently was amended and restated, effective July 1, 2021, and subsequently may be amended from time to time to make necessary and desirable changes.

​

The Plan is managed by a Board of Trustees comprised of both Local Union 725 and MCASF representatives. This site provides Participants with online access to complete information about your Healthcare Plan.

​

Health Plan Benefits

The MCASF Local 725 Health & Welfare Plan is your health plan; it is responsible for providing your benefits and setting rules for eligibility and coverage. To deliver these benefits efficiently, the Plan partners with specialized companies that administer specific services on its behalf. These vendors handle day-to-day operations such as processing claims, managing networks, and providing customer support, but the benefits themselves are funded and governed entirely by the Plan. Below are our partners in delivering your benefits.

Announcing a New Service...

The Board of Trustees are pleased to announce a new no-cost support service available to all members and their eligible dependents on the MCASF Local 725 Health Plan who have been diagnosed with cancer. The Health Plan has partnered with CancerNavigator to provide tailored education and guidance to cancer patients and their families as they navigate the many decisions that follow a cancer diagnosis. 

​

The CancerNavigator service is delivered by oncology-certified Nurse Navigators, backed by a team of researchers and data analysts. This team helps cancer patients:

​

  • Understand their specific diagnosis & treatment options

  • Discuss any questions related to their clinical situation

  • Schedule apppointments quickly with centers that are well-equipped to treat their specific cancer type

  • Prepare for upcoming doctor visits related to cancer

  • Access wraparound support for emotional & mental health, transportation, and community & family resources

Annual Tax Form

The W2 form for those members who received a Loss of Time disability in 2025 will be postmarked by January 31st.

If you do not receive your W2 by February 12th, you may request a reprint be done at that time. We are unable to reprint the form prior to that date,

The 1095B form for those on active health coverage in 2025 will be postmarked by March 2nd.

If you do not receive your 1095B by March 15th, you may request a reprint be done at that time. We are unable to reprint the form prior to that date

Annual Family Statement

To ensure that the MCASF Local 725 Health & Welfare Fund has up-to-date information on you and your family members, you are required to complete and return the Annual Family Statement each year beginning October 1st.

​

As self-funded, which simply means we pay for each claim submitted by participants, the Fund has an obligation to ensure that the plan assets are used appropriately, meaning for the sole benefit of eligible participants - Local 725 members and their eligible dependent family members.

​

The Family Statement had an extended deadline of December 30th with suspension of coverage and claims payment beginning January 1st.

​​

If you did not submit your statement, your coverage is now suspended. To get reinstated you can get a Annual Family Statement by clicking on the button or scanning the QR code below. You can also save time by completing your form online through your participant portal, click here.

​Be advised...eligibility files are sent every Wednesday morning, so you need to get your form in by Tuesday to get on that week's file. It will take Florida Blue at least 3 to 5 business after that Wednesday file to update their system and get you back on coverage.

Broward Health & Memorial Healthcare Systems
are still out-of-network with Florida Blue

Broward Health & Memorial Healthcare Systems have chosen to go out-of-network with Florida Blue. These entities are still negotiating to achieve a fair agreement.

However, until that occurs, all Broward Health & Memorial hospitals, physician groups and ancillary facilities are out-of-network for your plan.  Utilizing a Broward or a Memorial provider while out-of-network will result in a higher cost share for you.

While the Health Fund is not part of these negotiations, we encourage you to contact Florida Blue (800) 352-2583; Memorial Healthcare System (954) 276-7676; Broward Health (888) 551-4357 and ask these entities come to a resolution now as it has affected your ability to get the healthcare services you need.

On May 1, 2024, Sav Rx became the Health Fund's prescription benefit manager. You should provide your pharmacy with the Sav Rx ID card you received in the mail. This is a prescription insurance card, not a discount card.

Loss of Time
Benefit

Did you know that if you become totally disabled due to non-occupational bodily injury or sickness while eligible for benefits, the Health Fund will pay you a Loss of Time (Short-Term Disability) benefit.

​

Benefits will begin as of the first day of disability due to an accident or as of the 8th day of disability due to sickness and will continue for any one period of disability for a maximum of twenty-six (26) weeks. You do not have to be confined to your home to collect benefits, but must be under the care of a physician. 

​

For a  Loss of Time benefit , you will receive a benefit based on your job classification.

​

General Foreman, Foreman. R5, R1 & MESJ ~ $500.00 per week

R2 & Apprentice 5th Year ~ $ 360.00 per week

R3, R4, MES2, MES3, Apprentice 2nd year, 3rd year, 4th Year and MAT ~ $250.00 per week

Transparency in Coverage

Your Health Fund's compliance with the CCA and The No Surprises Act and Transparency in Coverage.

​

The Transparency in Coverage final rules require non-grandfathered group health plans such as our to disclose on a public website information regarding:

  • In-network negotiated rates for covered items and services

  • Out-of-Network allowed amount and billed charges for covered items and services

 

As of July 1, 2022, these machine-readable files (MRFs) must be made "publicly available and accessible to any person free of charge and without conditions, such as establishment of a user account, password or other credentials or PII to access the file" and must be updated on a monthly basis.

(These files are extremely large and download for an individual will be impacted by your hardware, browser and internet speed)​
